This is a powerful paper. ARUBA has given us important natural history data. But its over-application is causing very worrisome findings.

Interesting exploratory case series (unblinded/unadjudicated outcomes, not powered, retrospective) Notably, diameter is not an isolated variable -> varied catheter brands = variable trackability. Many early 071-074 did not track. IMHO: a signif reach to suggest a plateau effect

Great work! EVEREST, MIST, or whatever one calls it... creating parallel processes (physician travels while pt prepared) to decrease recanalization times results in improved outcomes.
Further indication that we have room to improve. Better technology, more navigable guide catheters to get beyond the tortuosity, larger bore aspiration catheters, etc.... We can do better.
